All participants will be given a gift card after enrollment in the study and completion of the baseline questionnaires. They will receive another gift card of $25 at the end of the study.

All participants will receive a transition newsletter approximately every 4 months containing resources and information regarding the transition process.

If randomized to the patient navigator, the participant will receive personal help and guidance during their transition from pediatric to adult care.

Usual care will be continued throughout the study from health care providers.
