
Alberta's Transition Navigator Trial

For Young Adults

Transition to adult care can be difficult for youth with chronic conditions.
The Transition Navigator Trial (TNT) is a study to evaluate the effectiveness of two interventions that assist young adult with transfer from pediatric to adult health care:

usual care, including access to generic transition resources.

usual care and patient navigator who will support youth for up to 24 months after transfer to adult care.


The patient navigator will help coordinate care by focusing on:

Preparation for transfer

Navigation of the healthcare system

Assisting in finding resources for non-health issues (e.g., services, housing, finance, transportation)

Promoting self-management of healthcare issues
This study will provide urgently needed data to better support youth and their families as they transition to adult care with possible improvements in:

Care delivery
-
Individual health outcomes
-
Patient experience of the healthcare system during transition
